Methadone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 2D6. Theoretically, schizonepeta might increase the effects and side effects of CYP2D6 substrates.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the idea behind this pairing. Lab and animal studies hint that a compound in Schizonepeta may slow down a liver enzyme called CYP2D6, which is one of the pathways your body uses to break down methadone. If that enzyme is working slower, methadone could clear out a bit less efficiently and build up. With an active opioid like methadone (not a prodrug, so it works as-is), more drug in your system can mean stronger sedation, slowed breathing, constipation, and low blood pressure.

I want to be straight with you, though: this is only a theoretical concern from early research, not something proven in people. Methadone is also a drug where small changes matter, so it's worth a quick chat with your pharmacist or doctor before combining them rather than guessing.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for methadone: your liver uses an enzyme called CYP3A4 to break it down. In animal studies, a compound in Schizonepeta appears to speed up (induce) that enzyme. If the same thing happened in people, your body could clear methadone a bit faster, which might weaken its effect. For pain control that could mean breakthrough pain, and for someone on a methadone program it could nudge you toward withdrawal feelings like restlessness, sweating, or aches.

I want to be honest though: this is only lab and animal data, so it's theoretical, not proven in humans. Methadone is a sensitive medicine where dose matters a lot, so please don't change anything on your own. Just loop in your pharmacist or doctor before adding Schizonepeta.

About Methadone

What Is Methadone Used For?

Brand name: Dolophine

Methadone treats severe and persistent pain that requires daily, around-the-clock, long-term opioid treatment and cannot be managed adequately with other options. It is available in several forms for this use, including oral solution, oral tablets, oral concentrate, and injectable forms (intravenous, intramuscular, and subcutaneous). Injectable methadone is not approved for outpatient pain treatment — it is reserved for hospitalized patients unable to take oral medication.

Methadone is also used for the detoxification and maintenance treatment of opioid addiction (dependence on heroin or other morphine-like drugs). Oral forms — including tablets, dispersible tablets (such as Methadose Dispersible and DISKETS), and oral concentrates (such as Methadone Hydrochloride Intensol™, Methadose, and others) — are approved for this use. By law, methadone for opioid addiction treatment must be dispensed only by certified opioid treatment programs. Injectable methadone is not approved for outpatient addiction treatment.
