Siltuximab falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the thing worth knowing about this particular pairing: the interaction on file is built around drugs that the liver enzyme CYP3A4 breaks down. Siltuximab (Sylvant), though, is a monoclonal antibody, a large protein. Your body doesn't clear it through CYP3A4 the way it does with small pills like statins or certain sleep medicines. So the direct enzyme concern this database entry describes doesn't really apply to Siltuximab itself.

On top of that, the evidence here is only from lab and animal studies, so it's theoretical even for the drugs it does touch. Realistically, I wouldn't expect Schizonepeta to weaken Siltuximab's effect on your Castleman's disease. Still, since this herb isn't well studied in people, please run it by your oncology team or pharmacist before adding it.

About Siltuximab Injection

What Is Siltuximab Injection Used For?

Brand name: Sylvant

Sylvant (siltuximab) injection is approved to treat multicentric Castleman's disease (MCD) — a rare disorder involving abnormal lymph node growth driven by an overactive immune signal called IL-6. It is specifically approved for patients who test negative for both HIV and HHV-8 (human herpesvirus-8).

Sylvant has not been studied in patients with MCD who are HIV-positive or HHV-8-positive, because it does not block the form of IL-6 produced by those viruses.
