Here's the honest picture: the interaction on file links Schizonepeta to drugs cleared by a liver enzyme called CYP1A2. The thing is, Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) isn't really a CYP1A2 drug. These three medicines are mainly handled by other pathways and transporters in your liver, so the specific concern in our database doesn't line up well with how Vosevi actually works in the body.
On top of that, this flag comes only from lab and animal research, not from people, so it's very theoretical. I wouldn't lose sleep over it. Still, because Vosevi is a serious hepatitis C treatment you want working at full strength, run any herbal supplement past your pharmacist or doctor before combining them, just to be safe.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the concern with mixing Schizonepeta and Vosevi (the sofosbuvir, velpatasvir, voxilaprevir combo you take for hepatitis C). One compound in this herb, schizonepetin, appears to speed up a liver enzyme called CYP3A4 in animal studies. Velpatasvir and voxilaprevir are partly cleared by that enzyme, so in theory, revving it up could lower their levels in your blood.
Why that matters here: with hepatitis C treatment, keeping drug levels steady is what clears the virus. Lower levels could mean a weaker effect and, in a worst case, the treatment not fully working. I want to stress this is theoretical and based only on lab and animal data, not real patients. Still, since a full cure is the goal, I'd skip the supplement during treatment and check with your pharmacist or doctor first.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?
Brand name: Vosevi
Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults without cirrhosis (liver scarring) or with compensated cirrhosis (mild liver scarring, Child-Pugh A). It is specifically for people who have already tried an HCV treatment that did not fully work.
Vosevi is approved for adults with HCV genotypes 1, 2, 3, 4, 5, or 6 who were previously treated with a regimen containing an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who were previously treated with sofosbuvir without an NS5A inhibitor. Note: Vosevi has not been shown to offer additional benefit over sofosbuvir/velpatasvir for adults with genotype 1b, 2, 4, 5, or 6 who received sofosbuvir without an NS5A inhibitor.
