Sesame and Pitavastatin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Why this applies to Pitavastatin

Pitavastatin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Sesame interacts with.

Theoretically, sesame might increase the levels and clinical effects of CYP2C9 substrates.
In vitro, sesame inhibits CYP2C9. However, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: in test tube studies, sesame appears to slow down a liver enzyme called CYP2C9, which plays a role in clearing pitavastatin from your body. If that enzyme gets blocked, pitavastatin could build up a bit more than usual. Since pitavastatin works as-is (it's not a prodrug that needs activating), more of it in your blood would mean a stronger effect and a slightly higher chance of statin side effects like muscle aches, soreness, or weakness.

That said, this is only a theoretical concern seen in the lab, never actually reported in people. Pitavastatin also relies less on CYP2C9 than many statins. Everyday sesame in food almost certainly won't matter. If you're taking concentrated sesame supplements, just mention it to your pharmacist or doctor to be safe.

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Pitavastatin page

“Liver strain runs through the serious end of pitavastatin's (Zypitamag, Nikita) supplement interactions, and this list deserves real care rather than a quick skim. The biggest cluster of worrisome entries is herbs and ingredients with their own record of liver injury, and alcohol belongs to that group too, with beer and wine both carry…”

About Sesame

What is Sesame?

  • Heart and cholesterol health
  • Blood pressure support
  • Source of healthy fats and minerals
  • Skin and hair care (topical oil)
  • Antioxidant support
  • Cooking and nutrition

Sesame is a nutritious seed and oil widely used in cooking and traditional medicine, and it provides healthy fats, fiber, and antioxidant compounds called lignans. Some early research suggests sesame may modestly help with cholesterol and blood pressure, but the evidence is limited. Sesame is also a common, sometimes serious, food allergen, so people with seed allergies should be careful.

About Pitavastatin

What Is Pitavastatin Used For?

Brand names: Zypitamag, Nikita

Pitavastatin tablets are used alongside a cholesterol-lowering diet to reduce LDL-C (low-density lipoprotein, or "bad" cholesterol) in adults with primary hyperlipidemia (high cholesterol not caused by another disease).

Livalo, Pitavastatin Calcium (tablet), and some other pitavastatin tablet products are also approved for adults and children aged 8 years and older with heterozygous familial hypercholesterolemia (HeFH) — an inherited condition that causes very high LDL-C from birth. Zypitamag is labeled only for adults with primary hyperlipidemia; its labeling does not include the pediatric HeFH indication.
