Sesame and Rosiglitazone Maleate

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Sesame interacts with whole categories of medication. Rosiglitazone Maleate falls into 2 of those categories, so it can interact with Sesame in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

1 of 2

Why this applies to Rosiglitazone Maleate

Rosiglitazone Maleate falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Sesame interacts with.

Theoretically, sesame might increase the levels and clinical effects of CYP2C9 substrates.
In vitro, sesame inhibits CYP2C9. However, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Avandia (rosiglitazone). Your liver uses an enzyme called CYP2C9 to break this medicine down. In test-tube studies, sesame appears to slow that enzyme a little, which could in theory let rosiglitazone build up slightly and push its effects up a notch. With a diabetes drug, that mostly means blood sugar dropping lower than expected, so watch for shakiness, sweating, dizziness, or feeling unusually tired.

The important part: this has only been seen in lab work, never reported in real people. Normal amounts of sesame in cooking, like on a bagel or in hummus, are very unlikely to matter. If you take concentrated sesame supplements, just mention it to your pharmacist or doctor so they can keep an eye on your numbers.

ANTIDIABETES DRUGS

2 of 2

Rosiglitazone Maleate also falls under ANTIDIABETES DRUGS.

Taking sesame oil with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical studies show that sesame oil can decrease plasma glucose and glycated hemoglobin (HbA1c) levels. Some clinical research in patients taking glibenclamide shows that using sesame oil or a blend of sesame oil and rice bran oil in place of other oil for cooking reduces plasma glucose more than glibenclamide alone. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with sesame and your Avandia (rosiglitazone): sesame oil on its own seems to nudge blood sugar down a bit, based on some early studies. Since rosiglitazone is already working to lower your blood sugar by making your body more sensitive to insulin, layering the two together could push your numbers lower than expected. That raises the chance of low blood sugar (hypoglycemia), which can feel like shakiness, sweating, dizziness, hunger, or a fast heartbeat.

That said, this is mostly theoretical and based on how sesame behaves in studies, not strong proof. A little sesame on your bagel or in stir-fry is unlikely to matter. Larger amounts, like cooking regularly with sesame oil or taking concentrated supplements, are more worth watching. Keep an eye on your readings and check with your pharmacist or doctor before making any changes.

From our pharmacist’s overview on the Rosiglitazone Maleate page

“Rosiglitazone (Avandia) lowers blood sugar in type 2 diabetes, and most of the 314 supplements and herbs on this page pull on that same lever. Some 239 can move blood glucose on their own, so adding one risks hypoglycemia, the shakiness, sweating, and confusion of sugar falling too low.”

About Sesame

What is Sesame?

  • Heart and cholesterol health
  • Blood pressure support
  • Source of healthy fats and minerals
  • Skin and hair care (topical oil)
  • Antioxidant support
  • Cooking and nutrition

Sesame is a nutritious seed and oil widely used in cooking and traditional medicine, and it provides healthy fats, fiber, and antioxidant compounds called lignans. Some early research suggests sesame may modestly help with cholesterol and blood pressure, but the evidence is limited. Sesame is also a common, sometimes serious, food allergen, so people with seed allergies should be careful.

About Rosiglitazone

What Is Rosiglitazone Used For?

Brand name: Avandia

  • Rosiglitazone is used along with a diet and exercise program and sometimes with one or more other medications to treat type 2 diabetes (condition in which the body does not use insulin normally and therefore cannot control the amount of sugar in the blood). Rosiglitazone is in a class of medications called thiazolidinediones. It works by increasing the body's sensitivity to insulin, a natural substance that helps control blood sugar levels. Rosiglitazone is not used to treat type 1 diabetes (condition in which the body does not produce insulin and therefore cannot control the amount of sugar in the blood) or diabetic ketoacidosis (a serious condition that may occur if high blood sugar is not treated).

  • Over time, people who have diabetes and high blood sugar can develop serious or life-threatening complications, including heart disease, stroke, kidney problems, nerve damage, and eye problems. Taking medication(s), making lifestyle changes (e.g., diet, exercise, quitting smoking), and regularly checking your blood sugar may help to manage your diabetes and improve your health. This therapy may also decrease your chances of having a heart attack, stroke, or other diabetes-related complications such as kidney failure, nerve damage (numb, cold legs or feet; decreased sexual ability in men and women), eye problems, including changes or loss of vision, or gum disease. Your doctor and other healthcare providers will talk to you about the best way to manage your diabetes.
