Soy and Hydrochlorothiazide, Losartan Potassium

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Soy interacts with whole categories of medication. Hydrochlorothiazide, Losartan Potassium falls into 2 of those categories, so it can interact with Soy in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTIHYPERTENSIVE DRUGS

1 of 2

Why this applies to Hydrochlorothiazide, Losartan Potassium

Hydrochlorothiazide, Losartan Potassium falls under ANTIHYPERTENSIVE DRUGS — the group Soy interacts with.

Theoretically soy protein may have additive effects with antihypertensive drugs and increase the risk of hypotension.
Although some contradictory research exists, most clinical evidence suggests that consuming soy protein modestly reduces systolic and diastolic blood pressure in individuals with prehypertension or hypertension.
Severity Moderate Likelihood Possible Evidence A

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Hyzaar pairs two blood pressure medicines: losartan, which relaxes your blood vessels, and hydrochlorothiazide, a water pill that helps your body shed extra fluid and salt. Soy itself can nudge blood pressure down a little, especially soy protein eaten regularly. So when you combine the two, the effects can add up and your pressure may drop a bit more than expected.

What that could feel like: dizziness, lightheadedness (especially standing up quickly), or feeling unusually tired. For most folks, normal soy foods like tofu, edamame, or soy milk are gentle and unlikely to cause trouble. Concentrated soy protein or isoflavone supplements are where I'd pay closer attention. Please don't change your dose on your own. If you want to start a soy supplement, check with your pharmacist or doctor first.

DIURETIC DRUGS

2 of 2

Hydrochlorothiazide, Losartan Potassium also falls under DIURETIC DRUGS.

Theoretically, soy might have additive effects when used with diuretic drugs.
Animal research suggests that genistein, a soy isoflavone, increases diuresis within 6 hours of subcutaneous administration in rats. The effects seem to be similar to those of furosemide. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Your Hyzaar combines two blood pressure medicines: hydrochlorothiazide, a water pill that helps your body get rid of extra fluid, and losartan, which relaxes your blood vessels. The concern here is pretty mild. In a rat study, a soy compound called genistein acted a little like a water pill, boosting urine output. The thought is that soy might add to the fluid-losing effect of your hydrochlorothiazide.

Keep this in perspective. This was only seen in animals and has never been reported in people, so it's theoretical. Eating normal soy foods like tofu or edamame is very unlikely to cause trouble. If you take concentrated soy isoflavone supplements, just mention it to your pharmacist or doctor, and watch for dizziness, lightheadedness, or feeling dehydrated.
