Acetaminophen, Butalbital, Caffeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
In your headache combo (Esgic, Fiogesic and similar), the part that overlaps with sulforaphane is the caffeine. Your body clears caffeine using an enzyme called CYP1A2. Sulforaphane has shown two opposite effects in lab and animal studies: it can slow this enzyme down (which could let caffeine linger and add jitteriness, a racing heart, or trouble sleeping), while its precursor can speed the enzyme up (which could make caffeine wear off faster).
Here's the honest part: none of this has actually been seen in people. It is theoretical, and the broccoli or veggies on your plate are very unlikely to matter. If you take a concentrated sulforaphane supplement, just mention it to your pharmacist or doctor so they can keep an eye on things.

Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

This headache combination leans on your liver to clear it out, and one of those liver enzymes is CYP3A4. In lab dishes and animal studies, sulforaphane has slowed that enzyme down. If that held true in people, the medicine (especially the butalbital part) could hang around a bit longer, which might mean more drowsiness, dizziness, or grogginess than usual.
Here's the honest part: this has only been seen in test tubes and animals, never confirmed in humans, so I wouldn't lose sleep over it. The broccoli on your dinner plate almost certainly won't cause a problem. A concentrated sulforaphane supplement is the bigger question, so run that by your pharmacist or doctor before combining them.

In vitro evidence shows that sulforaphane inhibits CYP2E1 enzymes. This interaction has not been reported in humans.

Here's the gist for your headache combo. The acetaminophen part is partly handled by a liver enzyme called CYP2E1. In test-tube studies, sulforaphane seems to slow that enzyme down. In plain terms, blocking CYP2E1 could mean acetaminophen is processed a little differently, and in theory that might shift how much of it lingers in your system.
I want to be honest about how soft this evidence is. This was only seen in lab work, never confirmed in real people, so I wouldn't lose sleep over it. Broccoli and other veggies on your plate are not a concern at all. A concentrated sulforaphane supplement is the only thing worth running by your pharmacist or doctor first, especially since acetaminophen is already easy to overdo.

Brand names: Tecnal, Esgic Plus, Fiogesic, Triad, Repan, Esgic, Fioricet
This combination product — available as capsules, tablets, and oral solution under various names including Fioricet — is used to relieve the symptoms of tension headaches (also called muscle contraction headaches).
All supported products, including Fioricet capsules and the oral solution, share this same indication. Because butalbital can be habit-forming, extended or repeated use is not recommended, and these products are not established as safe or effective for treating headaches that come back frequently over time.
