CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Acetaminophen, Chlorpheniramine, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This Coricidin product is a combo of three ingredients: acetaminophen (the pain and fever reducer in Tylenol), chlorpheniramine (an antihistamine), and dextromethorphan (a cough suppressant). The concern on file is that sulforaphane might tweak a liver enzyme called CYP1A2, which plays a minor role in handling these drugs. Here's the honest part: the evidence is very weak, coming only from lab and animal studies. Sulforaphane has not been shown to do this in real people.
In theory, if sulforaphane slows that enzyme, these drugs could linger a bit longer, possibly nudging up side effects like drowsiness, dizziness, or dry mouth. But broccoli on your dinner plate is unlikely to matter. If you're taking a concentrated sulforaphane supplement, just run it by your pharmacist first.

This Coricidin product is a combo: acetaminophen for fever and aches, chlorpheniramine (an antihistamine), and dextromethorphan for cough. The piece that matters here is dextromethorphan, which the liver partly processes using the CYP3A4 enzyme. In lab and animal studies, sulforaphane has slowed that enzyme down. If that held true in people, it could mean a bit more dextromethorphan hanging around, which might add to drowsiness, dizziness, or a foggy, lightheaded feeling, on top of the sleepiness chlorpheniramine already causes.
Here's the reassuring part: this is theoretical only and has never been reported in humans. The broccoli on your dinner plate is not the concern. If you take a concentrated sulforaphane supplement, just run it by your pharmacist or doctor first.

In vitro evidence shows that sulforaphane inhibits CYP2E1 enzymes. This interaction has not been reported in humans.

Here's the piece that matters in this combo product: it's the acetaminophen (the Tylenol part) that gets touched by an enzyme called CYP2E1. Lab studies suggest sulforaphane can slow that enzyme down. In a test tube, that's actually thought to reduce the formation of acetaminophen's harmful liver byproduct, not raise it, so the worry here is pretty mild.
The honest truth is this has only been seen in lab and animal work, never in real people, so I wouldn't lose sleep over it. Broccoli on your plate is no concern at all. If you're taking a concentrated sulforaphane capsule, just give your pharmacist a heads-up, and as always, don't exceed the acetaminophen on the label or pair it with alcohol.
