Why this applies to Acetaminophen, Dextromethorphan

Acetaminophen, Dextromethorphan falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might alter the levels and clinical effects of CYP1A2 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.

Here's the honest picture with this combo. The interaction on file involves an enzyme called CYP1A2, and sulforaphane (the active compound in broccoli) may either slow it down or speed it up depending on the lab study. Importantly, this has only been seen in test tubes and animals, never confirmed in people, so I wouldn't lose sleep over it.

For your cough and pain medicine, the dextromethorphan piece is mostly handled by a different enzyme (CYP2D6), and acetaminophen is cleared through other routes too, so CYP1A2 isn't the main player here. If anything shifted, you might notice slightly more drowsiness or dizziness, but that's theoretical. Eating broccoli is totally fine. If you take a concentrated sulforaphane supplement, just mention it to your pharmacist.

Here's the simple version for this cough and pain combo. The dextromethorphan piece is partly handled by a liver enzyme called CYP3A4. In test tube and animal studies, sulforaphane seems to slow that enzyme down. In theory, that could let a little more dextromethorphan hang around, which might mean more of its usual effects like drowsiness, dizziness, or feeling foggy. The acetaminophen part isn't really affected by this pathway.

The big caveat: this has only been seen in lab and animal research, never reported in real people, so I wouldn't lose sleep over it. Eating broccoli won't matter. If you take a concentrated sulforaphane supplement, just run it by your pharmacist or doctor first.

Here's the short version on combining sulforaphane with this Tylenol cough product. One of the enzymes that handles acetaminophen is called CYP2E1. In test-tube studies, sulforaphane appears to slow that enzyme down. The interesting twist is that CYP2E1 is actually the pathway that turns a small portion of acetaminophen into a harmful byproduct that can stress the liver. So in theory, gently blocking that enzyme could mean less of that toxic byproduct, not more buildup of the drug itself.

Honestly though, this is lab-only science, never shown in real people, and food amounts of broccoli won't move the needle. The dextromethorphan piece isn't really involved here. If you take a concentrated sulforaphane supplement, just give your pharmacist or doctor a heads-up so they can keep an eye on things.
