Acetaminophen, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with this combo cold-and-flu product (acetaminophen, dextromethorphan, and pseudoephedrine) and a sulforaphane supplement. The only real overlap is with the liver enzyme CYP1A2. Sulforaphane has shown two opposite effects in lab and animal studies: it can block this enzyme, but its precursor can also ramp it up. None of this has actually been confirmed in people.
For these three ingredients, CYP1A2 is a minor player at most, so any real-world change is likely small. In theory, if levels shifted, you might notice a bit more or less of the usual stuff, like drowsiness or dizziness from dextromethorphan, or jitteriness from pseudoephedrine. Eating broccoli is not a concern. If you take a concentrated sulforaphane capsule, just run it by your pharmacist.

Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

This combination cold and flu product mixes three medicines, and the one most likely tied to this note is dextromethorphan, the cough suppressant. Lab and animal studies suggest sulforaphane may slow down a liver enzyme called CYP3A4, which helps clear dextromethorphan. In theory, if that enzyme is blocked, dextromethorphan could build up a bit and lead to more drowsiness, dizziness, or feeling foggy.
Here's the honest part: this has only been seen in test tubes and animals, never reported in real people, so I wouldn't lose sleep over it. Broccoli on your plate is not a concern. Concentrated sulforaphane supplements are the bigger question mark, so check with your pharmacist before pairing them with these cold medicines.

In vitro evidence shows that sulforaphane inhibits CYP2E1 enzymes. This interaction has not been reported in humans.

Here's the piece that matters in this combo cold and flu medicine: the acetaminophen (Tylenol portion). Your liver uses an enzyme called CYP2E1 to process a small slice of acetaminophen, and that pathway can create a byproduct that's hard on the liver. In test tube studies, sulforaphane (the broccoli compound) actually slows that enzyme down. In theory, that could mean less of the harmful byproduct, not more, so this isn't a scary interaction.
Honestly, this has never been shown to be a real problem in people, and broccoli on your dinner plate won't move the needle. The bigger acetaminophen rule still stands: watch your total daily dose and avoid extra alcohol. If you take concentrated sulforaphane pills, just mention it to your pharmacist.
