Sulforaphane + Acetaminophen, Hydrocodone
Acetaminophen, Hydrocodone brand names: Vicodin, Vicodin ES, Vicodin HP, Lortab, Norco 10/325, and others.
Acetaminophen, Hydrocodone falls into 3 drug categories that Sulforaphane can interact with; the most serious is rated moderate. Theoretically, sulforaphane might alter the levels and clinical effects of CYP1A2 substrates.

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Sulforaphane interacts with whole categories of medication. Acetaminophen, Hydrocodone falls into 3 of those categories, so it can interact with Sulforaphane in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 3Why this applies to Acetaminophen, Hydrocodone
Acetaminophen, Hydrocodone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with your Norco or Vicodin (that's hydrocodone plus acetaminophen). The concern on file is that sulforaphane, the broccoli-derived compound, might nudge an enzyme called CYP1A2. In lab dishes and animals it can either slow this enzyme down or, through a related compound, speed it up. The catch is that CYP1A2 isn't the main pathway that handles this medication. Hydrocodone is mostly processed by other enzymes (CYP3A4 and CYP2D6), and acetaminophen has its own routes too.
So in real life, eating broccoli or other cruciferous veggies almost certainly won't change how this combo works. This is theoretical and only seen in test tubes, never in people. Still, if you're taking a concentrated sulforaphane supplement, just mention it to your pharmacist or doctor to be safe.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 3Acetaminophen, Hydrocodone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with your Vicodin or Norco (acetaminophen plus hydrocodone). The hydrocodone part is partly handled by a liver enzyme called CYP3A4. In lab and animal studies, sulforaphane appears to slow that enzyme down. In theory, that could let more hydrocodone hang around, which might mean stronger effects like extra drowsiness, dizziness, constipation, or slowed breathing.
The big thing to keep in mind: this is only seen in test tubes and animals, not in people, so it's a theoretical concern, not something proven. Broccoli and other veggies on your plate are very unlikely to cause trouble. A concentrated sulforaphane supplement is the part worth running by your pharmacist or doctor before you start.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES
3 of 3Acetaminophen, Hydrocodone also falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES.
In vitro evidence shows that sulforaphane inhibits CYP2E1 enzymes. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the interesting wrinkle with this combo. The acetaminophen part of Vicodin or Norco is partly handled by an enzyme called CYP2E1, and that pathway is the one that makes a small amount of a toxic byproduct that can stress the liver, especially at high doses. In lab dish studies, sulforaphane appears to slow down CYP2E1. In theory, that could mean less of that harmful byproduct, which isn't a bad thing on its own.
Honestly though, this hasn't been shown in real people, only in test tubes, so I wouldn't lose sleep over it. Broccoli on your plate is no concern at all. If you take a concentrated sulforaphane supplement regularly, just give your pharmacist a heads-up so we can keep an eye on things together.

“Someone taking hydrocodone with acetaminophen (Vicodin, Norco) for post-surgical or dental pain often reaches for a sleep aid, kava, or a glass of wine at night, and that is exactly where the risk sits. This combination calls for genuine vigilance.”

About Sulforaphane
What is Sulforaphane?
- Antioxidant support
- General cellular health
- Detoxification support
- Inflammation
- Skin and aging concerns
Sulforaphane is a natural compound found in broccoli and other cruciferous vegetables that has shown promising antioxidant and anti-inflammatory effects in lab studies, but strong human evidence for most specific health claims is still limited. It is generally well tolerated as part of a vegetable-rich diet, but if you take a concentrated supplement, talk to your pharmacist or doctor first.
