Sulforaphane and Acetaminophen, Phenylephrine, Chlorpheniramine

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Sulforaphane interacts with whole categories of medication. Acetaminophen, Phenylephrine, Chlorpheniramine falls into 3 of those categories, so it can interact with Sulforaphane in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Acetaminophen, Phenylephrine, Chlorpheniramine

Acetaminophen, Phenylephrine, Chlorpheniramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might alter the levels and clinical effects of CYP1A2 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the honest picture with this combo cold tablet. The interaction on file is about the liver enzyme CYP1A2, which plays a small role in processing some medicines. In lab and animal studies, sulforaphane has gone both ways: it can slow this enzyme down, but its broccoli-derived precursor can actually speed it up. None of this has actually been shown to happen in people.

For your Super Cold Tabs specifically, this enzyme is not the main route for acetaminophen, phenylephrine, or chlorpheniramine, so I wouldn't expect a real-world problem. Eating broccoli is a non-issue. If you take a concentrated sulforaphane capsule and notice extra drowsiness or jitteriness, just check in with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 3

Acetaminophen, Phenylephrine, Chlorpheniramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Here's the short version on pairing sulforaphane (from broccoli or a concentrated extract) with this cold combo product. The idea on file is that sulforaphane may slow down an enzyme called CYP3A4, which helps your body clear certain medicines. If that enzyme is blocked, those drugs can linger a bit longer, which in theory could mean a touch more of their usual effects, like drowsiness and dry mouth from the chlorpheniramine, or a little extra jitteriness or blood pressure bump from the phenylephrine.

The important part: this is theoretical, seen only in lab and animal studies, and it has not been reported in real people. Eating broccoli is no concern. If you take a high-dose supplement, just mention it to your pharmacist or doctor before combining.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES

3 of 3

Acetaminophen, Phenylephrine, Chlorpheniramine also falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES.

Theoretically, sulforaphane might increase the levels and effects of CYP2E1 substrates.
In vitro evidence shows that sulforaphane inhibits CYP2E1 enzymes. This interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the piece that matters in your cold tablet: the acetaminophen (Tylenol) part. One of the liver enzymes that handles acetaminophen is called CYP2E1, and in test-tube studies sulforaphane appears to slow that enzyme down. In theory, blocking it could change how acetaminophen is processed. Interestingly, that same pathway is what normally turns a small amount of acetaminophen into a harmful byproduct, so slowing it isn't clearly a bad thing.

The honest truth is that this has only been seen in lab dishes, never reported in real people. Broccoli and other veggies at the dinner table won't be an issue. If you take a concentrated sulforaphane supplement, just mention it to your pharmacist, and never exceed the acetaminophen dose on the label.

From our pharmacist’s overview on the Acetaminophen, Phenylephrine, Chlorpheniramine page

“Super Cold Tabs earns more caution than a cold remedy usually gets. It combines acetaminophen, the decongestant phenylephrine, and the antihistamine chlorpheniramine, and 14 of the 289 supplements on file interact at a major level, so those are the ones to understand first.”

About Sulforaphane

What is Sulforaphane?

  • Antioxidant support
  • General cellular health
  • Detoxification support
  • Inflammation
  • Skin and aging concerns

Sulforaphane is a natural compound found in broccoli and other cruciferous vegetables that has shown promising antioxidant and anti-inflammatory effects in lab studies, but strong human evidence for most specific health claims is still limited. It is generally well tolerated as part of a vegetable-rich diet, but if you take a concentrated supplement, talk to your pharmacist or doctor first.
