Why this applies to Bendamustine Hydrochloride
Bendamustine Hydrochloride falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Bendamustine is a chemotherapy drug given by infusion for certain leukemias and lymphomas, and the liver enzyme CYP1A2 plays a part in processing it. Here's the catch with sulforaphane (the active compound in broccoli): in lab dishes and animals it can both block and speed up this enzyme, so the direction of any effect is genuinely murky. If it slowed clearance, bendamustine could build up and worsen side effects like low blood counts, infections, nausea, and fatigue. If it sped things up, the drug might work less well.
The important part: none of this has actually been seen in people, and it's based only on test-tube and animal work. Eating broccoli is unlikely to matter, but a concentrated supplement during cancer treatment is worth running past your oncologist or pharmacist first.

What Is Bendamustine Injection Used For?
Brand name: Bendeka
Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.
For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
