Benzhydrocodone, Acetaminophen falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might alter the levels and clinical effects of CYP1A2 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.

Here's the honest picture with Apadaz (benzhydrocodone with acetaminophen) and sulforaphane. The concern on file is about CYP1A2, an enzyme sulforaphane may nudge in lab dishes and animals. But this isn't the main pathway that controls how your body handles this pain medicine, and the evidence is theoretical only. Nothing like this has actually been seen in people.

Sulforaphane is mixed in the lab: in some studies it slows CYP1A2, and its broccoli precursor may speed it up. That makes any real effect on your pain relief or side effects (drowsiness, constipation, slowed breathing) hard to predict and likely small. Food amounts of broccoli shouldn't matter. Before starting a concentrated supplement, just check with your pharmacist or doctor.

Here's the quick version for Apadaz (benzhydrocodone with acetaminophen). The benzhydrocodone part is actually a prodrug, meaning your body has to convert it into its active opioid (hydrocodone) before it can ease your pain. Lab and animal studies suggest sulforaphane may block an enzyme called CYP3A4, which is one of the pathways involved in handling this medication. In theory, slowing that pathway could let more of the active opioid hang around, which might add to typical opioid effects like drowsiness, dizziness, constipation, nausea, or slowed breathing.

Now, the honest part: this is only a theory from test-tube and animal research, and it has not been seen in actual people. Everyday broccoli on your plate is very unlikely to matter. A concentrated supplement is worth running by your pharmacist or doctor first.

Here's the practical picture with Apadaz (benzhydrocodone plus acetaminophen) and sulforaphane. The piece that matters here is the acetaminophen. In lab dishes, sulforaphane appears to slow down an enzyme called CYP2E1, and that's one of the pathways your liver uses to handle acetaminophen. CYP2E1 can turn a small amount of acetaminophen into a harmful byproduct, so in theory, gently quieting that pathway isn't necessarily a bad thing.

I want to be honest about how soft this evidence is. It comes only from test-tube work and has never been seen in real people, so I wouldn't lose sleep over it. Eating broccoli is no concern at all. If you take a concentrated sulforaphane supplement alongside Apadaz, just mention it to your pharmacist or doctor so they can keep an eye on things.

What Is Benzhydrocodone and Acetaminophen Used For?

Brand name: Apadaz

Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.

Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
