Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

This combo cough and cold product pairs chlorpheniramine (an antihistamine) with dextromethorphan (a cough suppressant). Both lean on liver enzymes, including CYP3A4, to be cleared from your body. Sulforaphane has shown in lab dishes and animal studies that it can slow CYP3A4 down. If that happened in people, these medicines could build up a bit and hit harder than expected, meaning more drowsiness, dry mouth, dizziness, or grogginess. Neither of these is a prodrug, so blocking the enzyme would boost their effect, not weaken it.

Here's my honest take: this is theoretical only and hasn't been seen in actual humans. Broccoli on your plate is no worry. If you take a concentrated sulforaphane supplement, just run it by your pharmacist first.

Here's the gist for this cough and cold combo (the antihistamine chlorpheniramine plus the cough suppressant dextromethorphan). Sulforaphane, the compound concentrated in broccoli, can block a liver enzyme called CYP2E1 in test-tube studies. If that enzyme helps clear these ingredients, blocking it could let them build up a little, meaning slightly stronger effects like extra drowsiness, dry mouth, dizziness, or a foggy, lightheaded feeling.

That said, this is a theoretical concern based only on lab work, not anything actually seen in people, so I wouldn't lose sleep over it. Broccoli on your dinner plate is not a problem. A concentrated sulforaphane supplement is the only real question mark, so check with your pharmacist or doctor before pairing the two.
