Cobimetinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Your body relies on an enzyme called CYP3A4 to break down cobimetinib (Cotellic) and keep its level in a safe range. Lab and animal studies suggest sulforaphane can slow this enzyme down. If that happened in people, cobimetinib could build up in your blood, which might mean more of its side effects, things like diarrhea, nausea, fatigue, skin rash, or changes shown on heart and eye exams your team already watches for.

Here's the reassuring part: this is only a theoretical concern from test-tube and animal work, and it hasn't been seen in humans. Eating broccoli and other veggies is fine. A concentrated sulforaphane supplement is the bigger question, so please run it by your oncology pharmacist or doctor before starting one.

About Cobimetinib

What Is Cobimetinib Used For?

Brand name: Cotellic

Cotellic (cobimetinib) is used in two ways. First, it treats adults with unresectable (cannot be surgically removed) or metastatic (spread to other parts of the body) melanoma that carries a BRAF V600E or V600K gene mutation — but only in combination with vemurafenib, another cancer drug. A tumor test must confirm the BRAF mutation before starting this combination.

Second, Cotellic is used on its own as a single agent to treat adults with histiocytic neoplasms, a group of rare cancers involving immune cells called histiocytes.
