CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Etonogestrel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
If you're using the etonogestrel implant (Implanon or Nexplanon), here's the honest picture. The concern on file is that sulforaphane, the compound from broccoli, might affect a liver enzyme called CYP1A2. But this is really shaky ground for etonogestrel, since it's mostly handled by a different enzyme (CYP3A4), so I wouldn't expect a meaningful change in how well your birth control works.
On top of that, the evidence here is only from lab dishes and animals, and the effects can even pull in opposite directions. Nothing like this has actually been seen in people. Eating broccoli and cruciferous veggies is not a worry at all. If you're thinking about a concentrated sulforaphane supplement, just run it by your pharmacist or doctor first to be safe.

Your etonogestrel implant (Implanon) releases a steady, low dose of hormone that your liver clears using an enzyme called CYP3A4. In lab and animal studies, sulforaphane appears to slow that same enzyme down. In theory, that could let a bit more hormone linger in your body, which might mean slightly stronger effects or hormonal side effects like spotting, breast tenderness, headaches, or mood changes.
Here's the reassuring part: this has only been seen in test tubes and animals, never reported in real people, so it's a very weak, theoretical concern. Eating broccoli and other cruciferous veggies is not a problem. If you're thinking about a concentrated sulforaphane supplement, just run it by your pharmacist or doctor first.
