Sofosbuvir, Velpatasvir, Voxilaprevir falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might alter the levels and clinical effects of CYP1A2 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.

Here's the honest picture on combining sulforaphane (the broccoli compound) with Vosevi. On paper, sulforaphane can nudge an enzyme called CYP1A2, which is one pathway that handles certain medicines. But the evidence here is very weak, coming only from lab dishes and animals, with nothing actually seen in real people. To make it even murkier, sulforaphane and its precursor seem to pull this enzyme in opposite directions, so there isn't a clear push one way or the other.

The main three ingredients in Vosevi aren't primarily cleared by CYP1A2 anyway, so I wouldn't expect a meaningful problem. Eating broccoli is fine. If you're thinking about a concentrated sulforaphane supplement during your hepatitis C treatment, just run it by your pharmacist or doctor first.

Here's the gist for Vosevi. Part of this combo, especially the voxilaprevir piece, is handled by a liver enzyme called CYP3A4. In test tube and animal studies, sulforaphane appears to slow that enzyme down. If that held true in people, the medication might clear a little slower and build up slightly, which could nudge up side effects like headache, nausea, tiredness, or diarrhea.

I want to be honest about the strength here: this is only lab and animal data, and it has never been reported in actual patients. Eating broccoli and other veggies isn't the concern. A concentrated sulforaphane supplement is the part worth running past your pharmacist or doctor before you start it during treatment.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults who do not have cirrhosis or who have compensated cirrhosis (mild liver scarring that is still functioning). It is specifically for people who have already been treated with an HCV regimen before and need a different option.

Vosevi is approved for adults with HCV genotype 1, 2, 3, 4, 5, or 6 who previously took a medicine that included an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who previously took sofosbuvir without an NS5A inhibitor.
