Swamp Milkweed and Bisacodyl, Docusate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

STIMULANT LAXATIVES

Why this applies to Bisacodyl, Docusate

Bisacodyl, Docusate falls under STIMULANT LAXATIVES — the group Swamp Milkweed interacts with.

Theoretically, excessive use of stimulant laxatives with swamp milkweed might increase the risk of toxicity.
Swamp milkweed contains cardioactive steroids known as cardenolides that may have digitalis-like activity. Excessive use of stimulant laxatives can cause diarrhea, resulting in potassium depletion. This might increase the risk of toxicity.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about this combo. Swamp milkweed contains heart-active compounds (cardenolides) that act a bit like the old heart medicine digitalis. Bisacodyl is a stimulant laxative, and if you lean on it too much it can cause watery diarrhea, cramping, and loss of fluids. That fluid loss can drag your potassium levels down.

Low potassium is exactly what makes those heart-active plant compounds more dangerous, so in theory the two together could raise the risk of irregular heartbeats or other toxicity. I'll be honest: this is a theoretical concern based on how these things work, not proven cases. Still, swamp milkweed isn't a safe supplement to begin with. Please run this by your pharmacist or doctor before combining them.

From our pharmacist’s overview on the Bisacodyl, Docusate page

“Oleander sits at the top of this page for good reason. Bisacodyl with docusate (Dulcolax Laxative Tabs) is a stimulant laxative, and overusing it depletes potassium, which makes the heart-affecting compounds in oleander far more toxic.”

About Swamp Milkweed

What is Swamp Milkweed?

  • Traditional digestive remedy
  • Diuretic (increasing urine)
  • Cough and respiratory complaints
  • Worm or parasite remedy (folk use)
  • Pollinator/garden plant rather than medicine

Swamp Milkweed is a native North American wetland plant best known today as a butterfly and pollinator garden plant, not a proven medicine. It contains toxic milky sap with cardiac glycoside-like compounds, and there is very little reliable human research to support any health use. Because of its potential toxicity, it should not be used as a supplement without professional guidance.
