Cortisone Acetate falls under CORTICOSTEROIDS — the group Swamp Milkweed interacts with.

Theoretically, taking swamp milkweed with prolonged corticosteroid therapy might increase the risk of toxicity.
Swamp milkweed contains cardioactive steroids known as cardenolides that may have digitalis-like activity. Long-term corticosteroid use can lead to potassium depletion and electrolyte imbalance, which can increase the risk of cardenolide toxicity.

Here's the connection worth knowing. Cortisone Acetate, like other corticosteroids, can lower your body's potassium when you take it for a long stretch. The compounds in Swamp Milkweed act a bit like digitalis (a heart medicine), and they become more dangerous to your heart when potassium runs low. So it isn't that the steroid changes how the plant is broken down. It's that the steroid can set up the exact electrolyte imbalance that makes the milkweed's heart effects more risky.

To be clear, this is a theoretical concern based on how these substances work, not on solid human studies. Still, given milkweed's real toxicity, I'd skip it and check with your pharmacist or doctor first.
