Theoretically, taking swamp milkweed with prolonged corticosteroid therapy might increase the risk of toxicity.
Swamp milkweed contains cardioactive steroids known as cardenolides that may have digitalis-like activity. Long-term corticosteroid use can lead to potassium depletion and electrolyte imbalance, which can increase the risk of cardenolide toxicity.

Here's the gist of this one. Swamp milkweed contains natural heart-acting compounds (cardenolides) that behave a bit like the old heart drug digitalis. The concern isn't that hydrocortisone changes how milkweed is broken down. Instead, it's about what hydrocortisone can do to your body chemistry over time. Taking hydrocortisone (Cortef) for weeks or months can slowly lower your potassium and shift your electrolytes.

Low potassium makes the heart more sensitive to those milkweed compounds, which could raise the risk of an irregular or unsafe heartbeat. I want to be honest that this is theoretical, based on how these things work rather than real human studies. Even so, swamp milkweed is genuinely toxic and not a proven medicine, so please check with your pharmacist or doctor before combining them.

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
