Cyclothiazide falls under DIURETIC DRUGS — the group Swamp Milkweed interacts with.

Theoretically, potassium depleting diuretics might increase the risk of toxicity with swamp milkweed.
Swamp milkweed contains cardioactive steroids known as cardenolides that may have digitalis-like activity. Potassium depletion can increase the risk of cardenolide toxicity.

Here's the simple version. Cyclothiazide is a thiazide-type water pill, and one of its well-known habits is flushing potassium out of your body along with extra fluid. Low potassium is the issue here. Swamp Milkweed contains heart-acting compounds (cardenolides) that work a bit like digitalis, and when your potassium runs low, those compounds can hit the heart harder than they should.

So the worry isn't that the milkweed changes how your diuretic works. It's that the diuretic can set the stage for milkweed toxicity, which could mean nausea, an irregular or unusually slow heartbeat, or feeling dizzy. This is theoretical, based on how the compounds behave, not on solid human studies. Still, Swamp Milkweed isn't a safe supplement, so please run it by your pharmacist or doctor first.
