Why this applies to Zanubrutinib

Zanubrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Brukinsa (zanubrutinib) and Sweet Annie. Your body relies on a liver enzyme called CYP3A4 to break down zanubrutinib. Sweet Annie's effect on that enzyme is genuinely mixed in lab studies: one of its compounds (artemisinin) may speed the enzyme up, while the whole-plant extract seems to slow it down. So we honestly can't predict which way things would tip in a real person.

If the enzyme sped up, your zanubrutinib could clear faster and work less well against your cancer. If it slowed down, levels could climb and raise the odds of side effects like bruising, bleeding, infections, or an irregular heartbeat. This is only test-tube evidence, not proven in people, but because zanubrutinib treats serious conditions, please run any Sweet Annie use by your oncologist or pharmacist first.

What Is Zanubrutinib Used For?

Brukinsa (zanubrutinib) is used to treat adults with several types of blood cancers. These include chronic lymphocytic leukemia (CLL), small lymphocytic lymphoma (SLL), Waldenström's macroglobulinemia (WM), mantle cell lymphoma (MCL) after at least one prior treatment, relapsed or refractory marginal zone lymphoma (MZL) after at least one prior anti-CD20 treatment, and relapsed or refractory follicular lymphoma (FL) — the last in combination with another medicine called obinutuzumab, after two or more prior treatments.

Some of these uses (MCL, MZL, and FL) have received accelerated FDA approval based on response rates, meaning they may be subject to further confirmation of long-term benefit in ongoing trials.
