Theoretically, fresh sweet clover might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Fresh sweet clover contains coumarinic acids, which are converted to free coumarins during drying. Dicoumarol, which has anticoagulant activity, also has been found in some sweet clover preparations and can be formed if fresh sweet clover is allowed to spoil.

Here's the thing to keep in mind: Cilostazol (Pletal) already works by making your platelets less sticky, which helps blood flow more easily through narrowed leg arteries. Sweet clover can add to that same effect. It contains coumarin, and if the plant spoils or is poorly dried, it can form dicoumarol, a compound with real blood-thinning activity. Stacked together, the worry is a higher chance of bruising or bleeding, like nosebleeds, bleeding gums, or stomach upset that doesn't quit.

I'll be honest, this concern is theoretical and based on how these compounds behave, not on strong human studies. Still, since both lean the same direction, I'd run it past your pharmacist or doctor before combining them rather than guessing.
