Why this applies to Teplizumab-mzwv

Teplizumab-mzwv falls under HEPATOTOXIC DRUGS — the group Sweet Clover interacts with.

Theoretically, concomitant use of sweet clover with hepatoxic drugs might increase the risk of hepatotoxicity.
Orally, large amounts of sweet clover can cause transient liver injury in susceptible individuals, possibly due to coumarin content. In one case report, a patient with multiple sclerosis developed jaundiced palms and a slight elevation in serum alanine aminotransferase (ALT) after taking sweet clover containing coumarin 10 mg daily for 3 years. Two weeks after starting therapy with subcutaneous interferon (IFN)-beta 1b, aspartate transaminase (AST) and ALT levels increased to 7 and 17 times the upper limit of normal, respectively. Levels normalized after discontinuation of both products. Subsequent use of IFN-beta 1a alone, without sweet clover, resulted in only a very slight increase in AST.

Here's the simple version. Teplizumab (Tzield) is an infused antibody given to help delay type 1 diabetes, and one of its known side effects is a temporary bump in liver enzymes. Sweet clover contains coumarin, which in large or spoiled doses can also stress the liver. The worry here is that stacking two things that both lean on the liver could add up and make liver irritation more likely.

I want to be honest about how weak this concern is for this exact pairing. It's theoretical, based mostly on how these substances behave, not on solid studies with Tzield. Teplizumab isn't a prodrug, so this isn't about it being activated or blocked. It's simply two liver stressors at once. If you're getting Tzield, please mention any sweet clover use to your doctor or pharmacist first so your liver labs can be watched.

What Is Teplizumab-mzwv injection Used For?

Brand name: Tzield

Tzield is used to delay the onset of Stage 3 type 1 diabetes (T1D) in adults and children aged 1 year and older who already have Stage 2 T1D — meaning their immune system is attacking insulin-producing cells, but they don't yet have overt high blood sugar symptoms.

Tzield is also used to slow the loss of the body's own insulin production in children and teenagers aged 8 to 17 years who have recently been diagnosed with Stage 3 T1D. This second use is approved under accelerated approval, meaning continued approval depends on results from ongoing confirmatory studies.
