Atropine, meperidine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2D6 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.

Here's the gist with meperidine (Demerol): your liver uses an enzyme called CYP2D6 to help process it. Lab studies suggest Terminalia arjuna may slow that enzyme down. In theory, that could let meperidine linger a bit longer, which might add to its usual effects like drowsiness, dizziness, constipation, or slowed breathing. With meperidine specifically, the bigger concern is a byproduct called normeperidine that can build up and, rarely, trigger tremors or seizures, so any change in how it's cleared is worth a careful eye.

Keep in mind this is only test-tube evidence, not proven in people, so the real-world effect may be small or none. Still, since meperidine is a strong opioid, please check with your pharmacist or doctor before combining them rather than changing anything yourself.
