Fruquintinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the idea behind pairing these two. Fruquintinib (Fruzaqla), your colorectal cancer pill, is partly cleared by a liver enzyme called CYP3A4. Lab studies suggest that Terminalia arjuna may slow that same enzyme down. If it does, your body could clear the drug more slowly, and levels might creep up. That could mean more of Fruquintinib's usual side effects, like high blood pressure, hand-foot skin reactions, mouth sores, tiredness, or diarrhea.

The honest part: this is based only on test-tube research, not real patients, so it may not play out this way in people. Still, since Fruquintinib is a serious cancer medicine, I'd loop in your oncology team or pharmacist before adding any Terminalia product rather than guessing.

What Is Fruquintinib Used For?

Brand name: Fruzaqla

Fruzaqla (fruquintinib) is used to treat adults with metastatic colorectal cancer (mCRC) — colorectal cancer that has spread to other parts of the body. It is specifically for patients who have already been treated with several standard chemotherapy regimens and other targeted therapies.

Fruzaqla is intended for patients who have previously received fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and — if their tumor is RAS wild-type and it is medically appropriate — an anti-EGFR therapy.
