Gilteritinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist for Xospata (gilteritinib). Your body relies on a liver enzyme called CYP3A4 to break this leukemia medicine down. Lab studies hint that Terminalia arjuna may slow that enzyme down, which could let gilteritinib build up a bit higher than expected. If that happened, you might see more of its known side effects, things like fatigue, muscle or joint aches, nausea, swelling, or in rarer cases heart rhythm changes (QT prolongation).

I want to be honest about the strength of this, though. It's only based on test-tube research, not real people taking both together, so we truly don't know how much it matters in practice. Since gilteritinib is a serious cancer drug, please don't start Terminalia without first running it past your oncologist or pharmacist.

About Gilteritinib

What Is Gilteritinib Used For?

Brand name: Xospata

Xospata (gilteritinib) is used to treat adults with acute myeloid leukemia (AML) — a fast-moving blood and bone marrow cancer — when the disease has come back after prior treatment (relapsed) or stopped responding to treatment (refractory). It is only used when a laboratory test approved by the FDA has confirmed that the leukemia cells carry a FLT3 mutation, a genetic change that helps the cancer grow.

Because response can take time, treatment for at least 6 months is generally recommended as long as the disease is not progressing and side effects are manageable.
