Irinotecan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist for irinotecan (Camptosar): your body uses an enzyme called CYP3A4 to help process this chemo drug. Lab studies (in test tubes, not people) suggest Terminalia arjuna may slow that enzyme down. If that happened in real life, irinotecan could build up a bit more than expected, which might mean stronger side effects like diarrhea, nausea, low blood counts, or fatigue.

I want to be honest about the strength of this: it's theoretical and based only on early lab work, so we don't truly know how much it matters in a person. Still, because irinotecan is a powerful cancer drug with a narrow safety margin, please don't add Terminalia without first running it by your oncologist or pharmacist.
