Mavacamten falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the situation with Camzyos (mavacamten) and Terminalia. Your body relies on a liver enzyme called CYP3A4 to break mavacamten down and clear it out. Lab studies hint that Terminalia arjuna may slow that same enzyme. If it does, mavacamten could build up higher than expected in your system.

Why that matters: mavacamten is a heart medication with a careful dosing window. Too much can weaken the heart's pumping action, leading to shortness of breath, swelling, dizziness, or fatigue. I want to stress that this concern comes only from test-tube research, not from people, so it's theoretical for now. Still, since mavacamten is so dose-sensitive, please check with your cardiologist or pharmacist before pairing it with this supplement.

It blocks CYP3A4, the liver enzyme that clears mavacamten, and too much of this drug can weaken the heart's pumping strength, the very problem careful dosing exists to prevent.”
