Osimertinib Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to help clear osimertinib (Tagrisso). Lab studies suggest Terminalia arjuna may slow that enzyme down a bit. If it does, your Tagrisso could clear more slowly and build up, which in theory might mean stronger effects and more side effects like diarrhea, mouth sores, rash, nail changes, or fatigue.

I want to be honest about how thin the evidence is. This comes only from test-tube research, not from people, so we really can't say it will happen in real life. Still, since osimertinib is a serious cancer medicine, please don't start Terminalia without first running it by your oncology pharmacist or doctor. They can tell you if it's worth the risk for you.

What Is Osimertinib Used For?

Brand name: Tagrisso

Tagrisso is used to treat adults with non-small cell lung cancer (NSCLC) whose tumors have specific EGFR mutations — either an exon 19 deletion or an exon 21 L858R mutation — confirmed by an FDA-approved test. It is used in several settings: after surgery to reduce the risk of cancer returning (adjuvant therapy), for locally advanced stage III NSCLC that has not progressed after chemoradiation, and as a first-line treatment for locally advanced or metastatic NSCLC (including in combination with pemetrexed and platinum-based chemotherapy).

Tagrisso is also approved for adults with metastatic NSCLC that carries a specific resistance mutation called T790M, when the cancer has stopped responding to another EGFR-targeted therapy.
