Quinine Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the basic idea: your liver uses an enzyme called CYP3A4 to break down quinine and clear it from your body. Lab studies suggest Terminalia arjuna may slow that enzyme down a bit. If quinine isn't cleared as quickly, its levels could creep up, which may mean stronger effects and a higher chance of side effects like ringing in the ears, headache, nausea, dizziness, or in larger amounts, heart rhythm changes.

That said, this concern comes only from test-tube research, not from people, so it's theoretical for now. I wouldn't panic, but quinine is a drug where blood levels matter, so it's worth a quick chat with your pharmacist or doctor before combining the two.
