Relugolix falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist for Orgovyx (relugolix): your body partly clears this medicine using a liver enzyme called CYP3A4. Lab studies hint that Terminalia arjuna may slow that same enzyme down. If it does, relugolix could clear out more slowly and build up a bit, which might mean more of its usual effects, like hot flashes, tiredness, joint pain, or loose stools.

I want to be honest about the strength of this, though. This warning comes only from test-tube research, not from people taking both together, so it's more of a theoretical caution than a proven problem. Since relugolix is an important cancer medicine, please don't add Terminalia on your own. Run it by your oncologist or pharmacist first so they can keep an eye on things.
