Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist: your body uses an enzyme called CYP3A4 to help clear aliskiren (Tekturna). Lab studies suggest that Terminalia arjuna may slow that enzyme down a bit. If that happens, a little more aliskiren could linger in your system, which in theory could push your blood pressure lower than expected. Watch for things like dizziness, lightheadedness when standing up, or feeling unusually tired.

Now, the important part: this is based only on test-tube research, not real people taking both together, so it's quite theoretical and may not amount to much. Still, since aliskiren already lowers blood pressure, I'd loop in your pharmacist or doctor before adding Terminalia, and keep an eye on your readings if you do.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.

Managing blood pressure is usually part of a broader plan that may also include diet changes, exercise, cholesterol control, and other medications. Many people need more than one blood pressure drug to reach their goal.
