Dolutegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Juluca combines two HIV medicines, dolutegravir and rilpivirine. The rilpivirine part is cleared mainly by an enzyme in your liver called CYP3A4. Lab studies suggest Terminalia arjuna may slow that enzyme down a bit, which in theory could let rilpivirine build up slightly. If that happened, you might notice more of its known effects, like headache, trouble sleeping, vivid dreams, or mood changes.

Here's the honest part though: this is only seen in test-tube research, not in real people, so it may not matter much in everyday life. Still, since keeping your HIV treatment steady is so important, I'd run any Terminalia supplement past your pharmacist or doctor before starting it rather than stopping or changing anything on your own.
