Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist for tadalafil (Cialis): your liver uses an enzyme called CYP3A4 to clear it from your body. Lab studies suggest that Terminalia arjuna may slow that enzyme down a bit. If that happens, tadalafil could hang around longer than usual, which means its effects and side effects could be stronger. With tadalafil, that might look like more flushing, headache, nasal congestion, an upset stomach, or a bigger drop in blood pressure (more dizziness or lightheadedness).

I want to be honest, though: this is only seen in test-tube research, not in real people, so it may not amount to much in everyday life. Tadalafil isn't a prodrug, so the direction is straightforward. Still, since both can affect blood pressure, please run this combo by your pharmacist or doctor before mixing them.

About Tadalafil

What Is Tadalafil Used For?

Brand names: Cialis, Tadliq

Tadalafil tablets (including Cialis and generic tadalafil) are used to treat erectile dysfunction (ED), the symptoms of benign prostatic hyperplasia (BPH — an enlarged prostate), or both conditions together. When used with the drug finasteride to start BPH treatment, this combination is recommended for up to 26 weeks.

Adcirca, Alyq (tablets), and Tadliq (oral suspension) are indicated specifically for pulmonary arterial hypertension (PAH — high blood pressure in the lungs), to improve the ability to exercise.
