Thioridazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2D6 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.

Here's the gist with these two together. Your body relies on a liver enzyme called CYP2D6 to clear thioridazine out of your system. Lab studies (done in test tubes, not in people) suggest Terminalia arjuna may slow that enzyme down. If that happens in real life, thioridazine could build up and hit harder than expected.

Why this matters for thioridazine specifically: it can affect your heart's rhythm (a QT prolongation risk), and higher levels could mean more drowsiness, dizziness, low blood pressure, or muscle stiffness. The evidence here is only theoretical right now, so I don't want to scare you. But because thioridazine is a sensitive drug, please loop in your pharmacist or doctor before adding Terminalia.

What Is Thioridazine Used For?

Brand name: Mellaril

Thioridazine Hydrochloride tablets are used for the management of schizophrenia — but only in patients who have already tried at least two other antipsychotic medications without adequate results, either because those drugs didn't work well enough or caused intolerable side effects.

Because thioridazine carries serious heart-rhythm risks, it is considered a last-resort option for schizophrenia, not a first-line treatment. It is not approved for dementia-related psychosis or any other psychiatric condition.
