Imipenem, Cilastatin falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Thuja interacts with.

Thuja products can contains significant amounts of thujone, a neurotoxin. Theoretically, patients taking drugs that lower the seizure threshold might be at greater risk of seizure if they also take thuja. Advise patients taking these drugs to avoid thuja products. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Here's the concern with pairing thuja and Primaxin (imipenem/cilastatin). Imipenem already carries a known risk of lowering your seizure threshold, meaning it can make seizures a bit more likely in some people, especially at higher doses or with kidney problems. Thuja's oil contains thujone, a compound that can act as a neurotoxin and may also nudge that threshold lower. Stack the two together and, in theory, the seizure risk could add up.

I'll be honest with you: this is based on anecdotal reports and the chemistry, not solid studies. Still, since seizures are serious, it's worth being cautious. Please don't change your antibiotic, but do check with your pharmacist or doctor before using any thuja product while on Primaxin.
