Thunder God Vine and Glofitamab-gxbm

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Thunder God Vine interacts with whole categories of medication. Glofitamab-gxbm falls into 2 of those categories, so it can interact with Thunder God Vine in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS

1 of 2

Why this applies to Glofitamab-gxbm

Glofitamab-gxbm falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Thunder God Vine interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of thunder god vine constituents.
Inhibitors of CYP3A4 can increase the concentration of triptolide, a thunder god vine constituent, and its derivative (5R)-5-hydroxytriptolide. In vitro research shows that triptolide and (5R)-5-hydroxytriptolide are about 94% and 64% metabolized by CYP3A4, respectively.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the thing to understand about this pairing: it's the Thunder God Vine that gets affected, not really Glofitamab (Columvi). Glofitamab is a lab-made antibody given by infusion, so your body clears it the way it handles proteins, not through the liver's CYP3A4 enzyme system. That means it doesn't behave like a typical CYP3A4 inhibitor drug even though it's grouped in that category.

The concern on file is theoretical and based on lab data only. If a drug slows CYP3A4, the herb's active compounds (like triptolide) could build up and cause more of its toxic effects. Since Thunder God Vine can already be quite toxic, and since Glofitamab is used for serious lymphoma, please loop in your oncology team before combining anything. Don't start or stop either one on your own.

IMMUNOSUPPRESSANTS

Large doses of thunder god vine may suppress immune function.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Glofitamab (Columvi) is an antibody treatment for certain lymphomas. It works by helping your own immune cells attack cancer cells, but part of that process can also weaken your body's normal immune defenses. Thunder God Vine contains compounds that, especially in larger doses, may also calm or suppress the immune system.

The concern here is additive. Stacking an immune-quieting herb on top of glofitamab could, in theory, leave you more open to infections or make immune-related side effects harder to manage. To be clear, this is a theoretical concern based on how these things work, not something proven in studies. Still, since glofitamab is a serious cancer therapy, please don't add Thunder God Vine without first checking with your oncologist or pharmacist.
