Glofitamab-gxbm falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with this pairing. The concern on file is that drugs which block a liver enzyme called CYP3A4 could slow the breakdown of modafinil (the active form your body makes from adrafinil). If modafinil hangs around longer, you might feel more of its stimulant effects, like trouble sleeping, a racing heart, jitteriness, headache, or anxiety.
The catch is that glofitamab-gxbm (Columvi) is a large antibody medicine given by infusion for lymphoma. Antibodies like this aren't broken down by CYP3A4 and generally don't meaningfully block it, so I wouldn't expect a real clash here. This warning is theoretical and really aimed at classic small-molecule CYP3A4 blockers. Still, adrafinil itself is unregulated and hard on the liver, so please run it by your oncology team before combining anything with your cancer treatment.
