Macitentan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Thunder God Vine interacts with.

Theoretically, thunder god vine might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that triptolide, a constituent of thunder god vine, inhibits CYP3A4 enzymes in human microsomes. This interaction has not been reported in humans.

Your body relies on an enzyme called CYP3A4 to clear Macitentan (Opsumit) out of your system. Thunder God Vine contains a compound (triptolide) that may slow this enzyme down. If that happens, Macitentan could build up a bit higher than usual, which might mean more of its typical side effects, things like headache, a stuffy nose, swelling in your legs or ankles, low blood pressure, or a drop in your red blood cell count (anemia).

I want to be honest about the strength of this concern: it comes only from lab studies, not from real people, so it's theoretical at this point. Still, since Thunder God Vine itself can be risky, please check with your doctor or pharmacist before combining the two, and don't change your Macitentan on your own.

What Is Macitentan Used For?

Brand name: Opsumit

Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a serious condition where the blood pressure in the arteries supplying the lungs is abnormally high. The goal is to slow down how quickly the disease gets worse and to lower the chances of being hospitalized because of it.

In clinical studies, it was shown to be effective in adults with mostly moderate-to-severe PAH symptoms. Patients in those studies had PAH caused by unknown or inherited reasons, connective tissue disorders, or congenital heart disease with repaired heart shunts.
