Palbociclib falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Thunder God Vine interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of thunder god vine constituents.
Inhibitors of CYP3A4 can increase the concentration of triptolide, a thunder god vine constituent, and its derivative (5R)-5-hydroxytriptolide. In vitro research shows that triptolide and (5R)-5-hydroxytriptolide are about 94% and 64% metabolized by CYP3A4, respectively.

Here's the thing to know about Ibrance (palbociclib) and Thunder God Vine: palbociclib is a mild blocker of the liver enzyme CYP3A4. Thunder God Vine's active compounds (like triptolide) are largely cleared by that same enzyme. So in theory, taking palbociclib alongside this herb could slow down the herb's breakdown and let its levels climb in your body.

That matters because Thunder God Vine can already be quite toxic if dosed wrong, causing things like stomach upset, diarrhea, and potentially liver or other organ harm. Higher levels could make those effects worse. I'll be honest, this is based on lab reasoning, not solid human studies, so it's more of a caution than a proven problem. Since you're already on cancer treatment, please loop in your oncologist or pharmacist before combining the two.

Here's the idea behind this pairing. Your body relies on a liver enzyme called CYP3A4 to break down and clear Ibrance (palbociclib). Thunder God Vine contains a compound (triptolide) that, in lab dish studies, slowed that same enzyme down. If that held true in people, palbociclib could build up higher than intended, which might mean more of its usual side effects, things like low blood counts (raising infection risk), fatigue, mouth sores, or nausea.

I want to be honest about the strength here: this is theoretical, based on test-tube data, and hasn't actually been reported in real patients. Still, palbociclib is serious cancer therapy, so don't add Thunder God Vine on your own. Please run it by your oncologist or pharmacist first.

Here's the thing to keep in mind if you're taking Ibrance (palbociclib) for breast cancer. This medicine already lowers your blood counts, especially your white blood cells, which is part of how it works but also leaves you more open to infections. Thunder God Vine has compounds that can calm or dampen the immune system too, particularly in larger, concentrated doses.

So the worry is overlap. Stacking an immune-calming herb on top of palbociclib could push your infection risk and low-count side effects further than expected. I'll be honest though, this concern is theoretical and based on how the two work, not on solid studies in people. Even so, with a cancer drug like this I'd play it safe. Please loop in your oncologist or pharmacist before combining them.

About Palbociclib

What Is Palbociclib Used For?

Brand name: Ibrance

Palbociclib (brand name Ibrance) is used to treat adults with hormone receptor (HR)-positive, HER2-negative advanced or metastatic breast cancer. It is always given in combination with other medicines — either an aromatase inhibitor as a first hormonal treatment, or with fulvestrant after the cancer has progressed on prior hormonal therapy. It is also used with inavolisib and fulvestrant for a specific subtype (PIK3CA-mutated, endocrine-resistant) of this cancer confirmed by an FDA-authorized test.

Ibrance is also approved in combination with trastuzumab (with or without pertuzumab) and endocrine therapy for maintenance treatment of adults with HR-positive, HER2-positive locally advanced or metastatic breast cancer, following initial induction treatment.
