Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs the other way from what you might expect. It is not that zonisamide changes your thyroid supplement much, but that taking the two together at the same time may get in the way of your body absorbing the thyroid hormone properly. Thyroid extract works best on an empty stomach, and swallowing it alongside other pills can blunt how well it is taken up.

The good news is this is rated a minor interaction, and the fix is simple: spacing. A common approach is to take your thyroid product first thing in the morning on an empty stomach and wait at least 30 to 60 minutes before other medicines like zonisamide. Keep your seizure control steady by never changing your zonisamide on your own, and check the best timing with your pharmacist or doctor.

What Is Zonisamide Used For?

Brand names: Zonegran, Zonisade

Zonisamide is used as add-on therapy — meaning it is combined with other seizure medications, not used alone — to treat partial seizures (seizures that start in one part of the brain) in adults with epilepsy. Zonegran (capsule) and generic zonisamide capsules are approved for adults with epilepsy.

Zonisade (oral suspension) is approved as add-on therapy for partial-onset seizures in adults and in patients 16 years of age and older.
