Belumosudil Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tree Turmeric interacts with.

Theoretically, tree turmeric might increase the levels and clinical effects of drugs that are substrates of CYP3A4.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, moderately inhibits CYP3A4.

Here's the gist for anyone taking Rezurock (belumosudil) alongside tree turmeric. Your liver uses an enzyme called CYP3A4 to clear belumosudil out of your system. Berberine, the active piece of tree turmeric, can moderately slow that enzyme down. When the cleanup crew works slower, more of the drug can hang around, and that may mean stronger effects and a higher chance of side effects like nausea, tiredness, swelling, muscle or joint aches, or changes in your blood counts and liver labs.

Belumosudil is not a prodrug, so this goes one direction: berberine could nudge its levels up. The concern here is mostly theoretical, but it's reasonable. Please loop in your pharmacist or transplant team before adding any berberine product so they can keep an eye on you.

Rezurock (belumosudil) is used to treat chronic graft-versus-host disease (chronic GVHD) — a serious immune condition that can occur after a stem cell or bone marrow transplant, where the donor's immune cells attack the recipient's body. It is specifically for patients aged 12 and older whose chronic GVHD has not responded to at least two prior systemic (whole-body) treatments.
