Here's the simple version: codeine is a narcotic pain reliever that already makes many people feel drowsy or foggy. Tree turmeric, mostly because of its berberine, has shown sedative-like effects in animal studies. So the worry is that the two together could pile on, leaving you sleepier, more lightheaded, or slower to react than codeine alone.

I want to be honest about the strength of this: it's theoretical and based on lab and animal work, not solid human studies, so it's not a hard 'never' situation. Food amounts of turmeric in cooking are very unlikely to matter. Concentrated berberine supplements are where I'd be more cautious. Don't change anything on your own, just check with your pharmacist or doctor first.

Theoretically, tree turmeric might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, can inhibit CYP2D6.

Here's the wrinkle with codeine: it's actually a prodrug. On its own it doesn't do much, your body has to convert it into morphine using an enzyme called CYP2D6, and that morphine is what relieves your pain. Tree turmeric (through its berberine) appears to slow that same enzyme down.

So in this case, blocking CYP2D6 likely means less of your codeine gets turned into morphine. The practical worry isn't more side effects, it's that your codeine may simply work less well, leaving your pain undertreated. This is based on early lab and limited human data, so it's a theoretical concern rather than a sure thing. Before pairing them, please check with your pharmacist or doctor.
