Wild Lettuce and Ipratropium

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICHOLINERGIC DRUGS

Why this applies to Ipratropium

Ipratropium falls under ANTICHOLINERGIC DRUGS — the group Wild Lettuce interacts with.

Studies report varying evidence on whether or not wild lettuce contains hyoscyamine, an anticholinergic drug. If it does, it appears to be in very low amounts. Nevertheless, there have been case reports of individuals consuming large amounts of wild lettuce experiencing anticholinergic-related side effects, such as constipation, dry mouth, dizziness, and sweating. Therefore, if you are already taking a drug with anticholinergic effects, you should be cautious about combining it with wild lettuce as the risk of side effects may be increased.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with ipratropium (Atrovent): it's an anticholinergic, meaning it dries up secretions, which is exactly how it helps open your airways. Wild lettuce might carry a tiny bit of its own anticholinergic-type activity, and there have been case reports of people taking large amounts ending up with classic anticholinergic effects like dry mouth, constipation, blurred vision, and dizziness.

So the concern is simply doubling up: stacking wild lettuce on top of ipratropium could nudge those same side effects a little higher. To be fair, the evidence here is weak and inconsistent, so this isn't an emergency. Still, if you use an inhaled or nebulized ipratropium daily and want to try wild lettuce, run it past your pharmacist or doctor first.

From our pharmacist’s overview on the Ipratropium page

“Ipratropium (Atrovent) opens the airways in COPD and asthma by blocking acetylcholine, and all 30 supplement and herbal ingredients on this page act on that same chemistry, which is why the list deserves genuine respect. The two rated major, henbane and scopolia, come first.”
