Desogestrel (Cerazette) is a progestogen-only birth control pill, so what these interactions put at stake is contraceptive failure, and I would treat this list with real care. Almost everything on it works through CYP3A4, the liver enzyme that clears desogestrel. Many ingredients slow that enzyme and raise hormone levels, but St. John's Wort does the reverse, speeding the drug's breakdown enough that pregnancy protection can slip, and that effect is documented.

Activated charcoal earns its major rating differently, by binding the pill in the gut so less gets absorbed at all, and grapefruit completes the trio of top concerns by pushing hormone levels up.
