ALC CLA contains two active ingredients: acetyl-L-carnitine HCl and safflower oil powder. Acetyl-L-carnitine is a compound that may support energy metabolism and brain function.
Safflower oil is extracted from the safflower plant and contains compounds that may affect blood clotting and blood glucose. The product also contains inactive ingredients — modified food starch, gelatin, calcium silicate, magnesium stearate, maltodextrin, titanium dioxide, and color additives FD&C Red #40 and FD&C Blue #1 — which serve as binders, fillers, and capsule material.

The evidence supporting acetyl-L-carnitine for its marketed uses comes largely from animal and laboratory research. Our data rates it as possibly effective for age-related cognitive decline, Alzheimer disease, alcohol use disorder, diabetic neuropathy, and depression — meaning there's some evidence but it's not conclusive.
For safflower oil, the evidence is weaker still; we hold insufficient reliable evidence to rate its use for unstable angina, coronary heart disease, cystic fibrosis, diabetes, or diabetic kidney disease. How safe is it?

Acetyl-L-carnitine is generally well tolerated over the short term, but long-term safety isn't fully established. Common side effects include agitation, dry mouth, headache, insomnia, reduced appetite, and a fishy odor in urine, breath, or sweat.
In people with Alzheimer disease, psychiatric effects like depression, mania, confusion, or aggression have been reported, though it's unclear whether they're from the supplement or the condition itself. Safflower oil in normal food amounts is well tolerated, but safflower supplements — especially at high doses — have less safety data.
Concerning case reports link safflower overuse to liver failure, though the exact dose and duration that caused harm weren't always clear. The safety data advises against acetyl-L-carnitine in pregnancy and during breastfeeding.
For safflower, safety during breastfeeding is unknown beyond normal food use, so medicinal amounts are best avoided.

Safflower Oil powder
Anticoagulant/Antiplatelet Drugs
High doses of safflower oil might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
Small clinical studies show that taking safflower oil, approximately 55 grams daily for 2-3 weeks, decreases platelet aggregation. However, taking lower doses of safflower oil, such as 5 grams daily for 4 weeks, does not seem to affect platelet function. In one case report, a 74-year-old male stabilized on warfarin developed urinary tract bleeding and an elevated INR after taking a safflower extract 20 grams daily for 14 days.
Antidiabetes Drugs
Theoretically, safflower oil might alter the effects of antidiabetes drugs.
Some clinical research shows that taking safflower oil 10 grams daily for 3 weeks can increase fasting blood glucose in patients with type 2 diabetes. However, clinical research in patients with metabolic syndrome with or without impaired glucose tolerance shows that taking safflower oil 8 grams daily for 12 weeks reduces fasting glucose levels by around 8 mg/dL. Some clinical research also shows that taking safflower oil 8 grams daily for 16 weeks does not affect fasting glucose levels in patients with type 2 diabetes.
Warfarin
Theoretically, safflower oil might increase the risk of bleeding when taken with warfarin.
In one case report, a 74-year-old male stabilized on warfarin developed urinary tract bleeding and an elevated INR after taking a safflower extract 20 grams daily for 14 days.
Acetyl-L-Carnitine HCl
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine, the parent compound of acetyl-L-carnitine, might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with acetyl-L-carnitine and warfarin.
