The evidence varies by ingredient and use. Saccharomyces boulardii shows possibly effective ratings for travelers' diarrhea, rotaviral diarrhea, antibiotic-associated diarrhea, Clostridioides difficile infection, necrotizing enterocolitis, and Helicobacter pylori.
Bacillus coagulans is rated possibly effective for constipation and irritable bowel syndrome (IBS). Bacillus subtilis is possibly effective for antibiotic-associated diarrhea.
For other uses listed — athletic performance, cancer prevention, liver disease — the evidence in our data is insufficient to rate these ingredients. We don't hold effectiveness data for Bacillus clausii, Lactobacillus LB Fermentate, or Triphala Fruit Extract.
How safe is it?
Well-documented data
These probiotic strains are generally well tolerated in healthy adults, but the product carries important cautions. Live microorganisms in probiotic supplements can very rarely cause serious infections (bacteremia or sepsis) in critically ill patients, those with central lines or feeding tubes, or people with weakened immune systems — preterm infants and immunocompromised patients should avoid this product.
Saccharomyces boulardii has rarely caused gastrointestinal complaints like cramping, bloating, nausea, or decreased appetite. Bacillus subtilis caused a headache in one trial participant and diarrhea has been reported rarely.
Overall, safety data is limited; talk with your doctor before starting, especially if you're very ill or have a weakened immune system.

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
