Fem contains 12 ingredients, including six digestive enzymes: amylase, protease, lipase, cellulase, glucoamylase, alpha-galactosidase, and hemicellulase. These are designed to help break down food during digestion.
The product also includes a proprietary enzyme blend, medium chain triglyceride powder (a type of fat), and three herbal ingredients: dong quai, wild yam, and chaste tree (also called vitex). These herbs are traditionally used for women's health, particularly around the menstrual cycle and menopause.
Inactive ingredients (fillers and binders) include cellulose, water, and phytase.
Does it work?
Moderate evidence
The digestive enzymes in this product—protease, lipase, and the others—have insufficient reliable evidence to establish their effectiveness for conditions like indigestion (dyspepsia) or inflammatory bowel disease. The herbal ingredients show mixed evidence: chaste tree is possibly effective for premenstrual syndrome (PMS) and breast tenderness (mastalgia), but wild yam is possibly ineffective for menopausal symptoms.
Dong quai and wild yam lack solid evidence for the conditions they're marketed for, including anemia, bone health, heart disease, and migraines. The safety data we hold does not establish that this product works for any specific condition.
How safe is it?
Well-documented data
The digestive enzymes—protease and lipase—are generally well tolerated in adults, though some people report digestive upset or allergic reactions. Protease occupational exposure has rarely caused allergic reactions in workers.
Lipase has no reported adverse effects in adults, but was linked to serious intestinal problems in premature infants given it in formula (a use not relevant to this product for adult consumers). Dong quai may cause burping and flatulence; it increases sun sensitivity, so protect your skin from sunlight.
Wild yam is generally well tolerated but may cause fever, headache, upset stomach, vomiting, or rarely anaphylaxis. Chaste tree commonly causes diarrhea, fatigue, headache, insomnia, irregular menstruation, nausea, skin irritation, stomach pain, or vomiting, and skin conditions like itching, rash, acne, and hair loss have been reported.
Mojave yucca's supplement safety is not well studied. None of these ingredients should be used in pregnancy or while breastfeeding—the safety data advises against it.

If you take warfarin or any other blood thinner or antiplatelet medication (like aspirin, clopidogrel, or dabigatran), do not start this product without checking with your doctor or pharmacist first — dong quai significantly raises bleeding risk. Also double-check before taking this if you use estrogen therapy or oral contraceptives (birth control pills), since dong quai, wild yam, and chaste tree may reduce their effectiveness.
Similarly, check if you take metoclopramide (nausea/reflux medication), antipsychotics, or dopamine agonists (Parkinson's drugs) — chaste tree may interfere with how they work.

Dong Quai
Warfarin (Coumadin)
Dong quai may increase the risk of bleeding when used with warfarin.
Case reports suggest that concomitant use of dong quai with warfarin can increase the anticoagulant effects of warfarin and increase the risk of bleeding. In one case, after 4 weeks of taking dong quai 565 mg once or twice daily, the international normalized ratio (INR) increased to 4.9. The INR normalized 4 weeks after discontinuation of dong quai.
Anticoagulant/Antiplatelet Drugs
Theoretically, dong quai may increase the risk of bleeding when used with anticoagulant or antiplatelet drugs; however, research is conflicting.
Animal studies suggest that dong quai has antithrombin activity and inhibits platelet aggregation due to its coumarin components. Additionally, some case reports in humans suggest that dong quai can increase the anticoagulant effects of warfarin. However, clinical research in healthy adults shows that taking 1 gram of dong quai root daily for 3 weeks does not significantly inhibit platelet aggregation or cause bleeding. Until more is known, use dong quai with caution in patients taking antiplatelet/anticoagulant drugs.
Estrogens
Theoretically, dong quai may reduce the effects of estrogens.
Dong quai has estrogenic effects. Theoretically, concomitant use of large amounts of dong quai might interfere with hormone replacement therapy due to competition for estrogen receptors.
Chaste Tree
Antipsychotic Drugs
Theoretically, vitex agnus-castus could interfere with the activity of antipsychotic drugs.
Vitex agnus-castus might interfere with the action of dopamine antagonists such as antipsychotic drugs due to its dopamine agonist effects.
Contraceptive Drugs
Theoretically, vitex agnus-castus could interfere with oral contraceptives.
Vitex agnus-castus might interfere with the efficacy of oral contraceptives due to possible hormone modulating activity.
Dopamine Agonists
Theoretically, vitex agnus-castus could interfere with dopamine agonists.
Vitex agnus-castus might potentiate the actions of dopaminergic agonists due to possible dopaminergic effects.
Estrogens
Theoretically, vitex agnus-castus could interfere with the activity of estrogens.
Vitex agnus-castus has hormone modulating activity that can interfere with the efficacy of hormone replacement therapy.
Metoclopramide (Reglan)
Theoretically, dopaminergic effects of vitex agnus-castus could interfere with metoclopramide.
Vitex agnus-castus might interfere with the action of dopamine antagonists such as metoclopramide due to its possible dopaminergic effects.
Wild Yam
Estrogens
Theoretically, wild yam might increase or decrease the effects of estrogen.
Wild yam root shows estrogenic and anti-estrogenic effects in vitro. Theoretically, wild yam might interfere with hormone therapy.
